A Call to Action: Promoting Effective Care Across the Health Continuum

Ensuring effective and appropriate care across the health continuum has always been a priority of the health system.  Now, more than ever, health care organizations – from hospitals to community care access centres (CCACs), home and community care providers – are required to work collaboratively and creatively to provide patient/client care. Provincial expert panels, evolving policy frameworks and increased requirements for data and evidence-supported outcomes are driving fundamental change within the health care system.

Plenary and concurrent sessions interwoven throughout the forum provide an opportunity to examine and discuss fundamental issues such as:

 

• Working to improve the care journey
• Advancing policy directions
• Evolving funding and financial frameworks
• Caring for high risk individuals
• Accelerating care transitions
